CCAP provides a variety of programs to assist individuals and families with their housing needs if they are homeless or at risk of homelessness
Housing Programs
All housing programs prioritize households based on vulnerability level determined at Coordinated Entry.
Whenever possible, households experiencing a housing crisis should be diverted from entering homeless housing programs through problem-solving conversations, linkages to mainstream and natural supports, and/or flexible, and light-touch financial assistance. Initial assessment and services address the immediate housing crisis with the minimal services needed. Frequent re-assessment determines the need for additional services.
- Services are individualized and responsive to the needs of each household.
- Households exit to permanent housing as soon as possible. Having already received assistance does not negatively impact a household’s eligibility if they face homelessness again.
- Households that are receiving housing assistance are required to participate in Care Coordination Services to work on their housing stabillity.
Homeless Housing Assistance
County: Grays Harbor and Pacific County
Eligibility: At or below 80% HUD income guidelines. Unsheltered homeless, Sheltered Homeless, or actively fleeing Domestic Violence.
Services Provided: (Required)Care Coordination Services, Move-in costs, abd temporary rental assistance to rapidly move households into more permanent housing solution.
At Risk Housing Programs
County: Grays Harbor and Pacific County
Eligibility: At or below 80% HUD income guidelines. Must be behind in rent/space rent for at least current month.
Services Provided: (Required)Care Coordination Services, past due rent/space rent, court fees incurred by landlord Utility only assistance if it will prevent eviction.
HEN
County: Grays Harbor County, Pacific County, and Lewis County
Eligibility: Individuals that are Homeless or At Risk of homelessness and have a HEN referral provided by DSHS.
HEN packs are available once per calendar month for clients on HEN/ABD program through DSHS

Jail Re-entry Program
County: Grays Harbor
Eligibility: Incarcerated in Grays Harbor County Jail or released from there in the last 24 hours, in need of clean and sober living, and has a substance use disorder.
Services Provided: Care Coordination Services, Weekly Support Groups, and time limited clean and sober housing
SUD Peer Program
Working with Summit Pacific ED, SUD Peers receive referrals from the ED for patients with current or past SUD needs.SUD Peers focus on assisting patients in getting connected to treatment, MAT clinics and recovery groups. SUD Peers can also assist patients in connecting with medical care, housing services and other community resources.
Youth and Young Adult Housing Programs
YHDP RRH & SSO (Supportive services)
County: Grays Harbor and Pacific County.
Eligibility: Homeless Young adults between the age of 18-24 years old
Services Provided: (Required)Care coordination and monthly housing costs for up to 24 months.
IYHP
County: Grays Harbor and Pacific County
Eligibility: Individuals who are between 18-24 years old who were a dependent of Washington State or a federally recognized tribal foster care program prior to the age of 18, or who are enrolled in Extended Foster Care
Services Provided: (Required)Care coordination, monthly housing costs, and Supportive services for costs related to housing stability
Referral for FYI vouchers
County: Grays Harbor and Pacific County
Eligibility: Individuals 18-24 who have left or are leaving foster care within 90 days and are homeless or at risk of homelessness. Including Extended Foster Care (Will lose Extended Foster Care Benefits once enrolled)
Services Provided: Refferal to Housing Authority for housing voucher for those in foster care or extended foster care and Supportive services for costs related to housing stability
Youth and young adults RRH housing program
County: Grays Harbor
Eligibility: At Risk of Homelessness or Homeless youth between the ages of 18 and 24 that are within income guidelines.
Services Provided: (Required) Care coordination and monthly rental costs
