Care Coordination Specialist, Pacific County

SUMMARY DESCRIPTION: This position provides comprehensive care coordination for individuals experiencing homelessness or housing instability. The Care Coordinator works with participants to develop individualized support plans to achieve housing stability and move toward self-sufficiency. The Care Coordinator interacts with diverse community supports, including behavioral health, substance abuse treatment, physical health, criminal justice, benefits management and employment.

This is a full-time, hourly position. The position is based in Pacific County WA, but may occasionally require travel to and work in our Grays Harbor and Pacific County offices. The primary office location will be in Longbeach, WA.

SUMMARY DESCRIPTION: This position provides a specific intervention to eligible DSHS Long-Term Care clients that may have multiple barriers to housing stability, including disabilities and substance abuse. This position will be responsible to assist eligible clients locate housing that is affordable, independent, permanent, and help them sustain their tenancy with a tenant-centered approach. This is a full-time position located in Olympia, Washington.

SUMMARY DESCRIPTION:  This position is primarily responsible for assisting eligible clients’ transition from an institutional setting to a home and community setting. This position will also be responsible to support the GOSH team, as asked/delegated, to eligible DSHS Long-Term Care clients that may have multiple barriers to housing stability, including disabilities and substance abuse. This position will be responsible to assist eligible clients locate housing that is affordable, independent, permanent, and help them sustain their tenancy with a tenant-centered approach.  This is a full-time position located in Aberdeen, Washington serving Grays Harbor and Pacific Counties while also providing support to Thurston, Mason, and Thurston Counties, as needed.

WorkFirst Case Manager

SUMMARY DESCRIPTION: This position is responsible for all aspects of the Workfirst program including: Job coaching, contract compliance, site development, maintenance of caseload, file maintenance, coaching participants at their worksite, providing assistance to employers and participants in addressing barriers to employment and ensuring all programs rules are followed carefully. This is a full-time position and depending on location of the preferred candidate, this position will be located in either our Olympia or Centralia office.

HEAD COOK – pORT aNGELES

SUMMARY DESCRIPTION:  We are seeking an experienced, kind, passionate, dependable cook to prepare meals five days per week for our Older Adults in Clallam County at our Port Angeles site.

FUNCTIONAL RESPONSIBILITIES:

  1. Prepare, cook, and serve meals for approx. 30 to 60 seniors per day, from a pre-established menu.
  2. Help facilitate the home bound meals program, and provide information regarding services to clients and families.
  3. Provide support for the Senior Nutrition Coordinator and Program Specialist.
  4. Help develop training programs for volunteers and trainees, delivery of training for volunteers and trainers.
  5. Participate in bi-monthly cooks meetings.
  6. Recruit volunteers for the particular site to assist with all aspects of the Senior Nutrition Program in Clallam County.
  7. Ordering for the particular site
  8. Reconciling of cash donations daily and deposit weekly
  9. Maintain high levels of sanitation and cleanliness standards
  10. Follow all local, state, and federal regulations in prepping, storing, and cooking food daily.
  11. Other duties as assigned.
